CPS Reports:
· Call CPS at (209) 468-1330. Press 4 to make a report, Press 4 to indicate your mandated reporter status.
· If technology is available, print, scan and email your CPS report to the SW taking the report (if they are willing to provide their email).
· If the SW is not willing to give you their email or if scanning/emailing is not available, send report by mail to CPS Attn: (name of SW) PO BOX 201056 Stockton, CA 95201.
Special considerations:
When completing a CPS report, Reporters Address should be the school site of the student. Since CPS cannot meet with students at school they are requesting we provide all available addresses for the student’s location, and all available contact numbers for the guardians.
